Eligibility Requirements
- Applicant has an implemented employee handbook and guidelines
- Applicant provides sexual harassment and discrimination awareness training to its employees/managers
Submission Requirements
- Signed and dated application
- Five year loss history, including a supplemental claim form completed for each loss
- Third party coverage supplement questionnaire (if coverage desired)
