Submission Requirements
- 1-9 Drivers (Truckers only):
- - Completed and signed Plan A, B, or C enrollment form for each driver
- 10+ Drivers:
- - CL application
- - Copy of lease agreement
- - Copy of safety/driver manual
- - Details for any loss over $25,000
- - Group application
- - List of drivers by state with date of birth
- - 3 years loss runs
