Submission Requirements
- Completed and signed ACORD 130
- Completed and signed TSP Supplemental Questionnaire
- Three years of currently valued (within 90 days of effective date) detailed loss runs
- Three years of detailed payroll history
- Copy of (a) employment application, (b) employee time card, (c) contract between insured and temporary worker, (d) written safety material, training program, return to work program, etc.
- Sales/marketing brochure and related company information
- Resume of each owner or company principal
